We have an electronic patient form that will allow you to enter all your medical information on a secure site that will transfer the information directly into our chiropractic software program.
The link will take you to a second site where you will see at fillable form that looks like the graphic to the right, and says Kempsville Chiropractic with our logo.
The form has different sections as you scroll down it. If you are filling out the form for your child, you will also need to fill out the pediatric specific section that starts about 1/2 of the way down the form.

The purpose of this consultation is to review and assess your medical history, discuss your current condition, and then decide the necessary steps to take next. This free service is highly recommended to those new to chiropractic care, and gives you, the patient, the opportunity to meet with one of our doctors. What You Can Expect With Your First Appointment
At Kempsville Chiropractic, we will never make an initial adjustment without first making an accurate assessment of your condition. To understand what the underlying cause of your problem is, we will test reflexes, do sensory testing, and various orthopedic tests. Depending on your individual circumstance, we can do a full examination with orthopedic, neurological, chiropractic, and physical exam tests, as needed, to check and determine the underlying cause of your problem. If necessary we will take x-rays. An x-ray of the spine allows us to determine exactly what your spinal alignment looks like, see if there is any disc degeneration, as well as look to see if there is any osteoarthritis which can be effecting or putting pressure on the nerves.

Based on the results of your consultation and exams, we are now ready to make the necessary adjustments to relieve your pain and correct the source of your problem. Chiropractic adjustments are gentle corrections that move your spine back into proper alignment so you get relief from pain, and your body is allowed to heal.
